Good Faith Estimate (No Surprises Act)

Under the No Surprises Act, you have the right to receive a “Good Faith Estimate” explaining the expected costs of your mental health care.

This estimate outlines the anticipated charges for therapy services if you are uninsured, not using insurance, or choosing to self-pay.

You can request a Good Faith Estimate at any time—before scheduling, during your intake process, or at any point throughout your treatment. We will provide this information in writing and answer any questions you may have about fees or the cost of care.

If you receive a bill that is at least $400 more than your Good Faith Estimate, you have the right to dispute the charge.

For more information, visit www.cms.gov/nosurprises or call 1-800-985-3059.

We are committed to transparency and ensuring you feel fully informed about the financial aspect of your care.

This Good Faith Estimate does not obligate you to receive care from our practice.
